This all depends on how the BIOS presents the USB drive to the software on
the flash drive. What's on the CD and the flash drive expects to be
booted from a floppy in drive A:. I suspect the USB drive shows up a
drive C: or something similar. This means you need code in the MBR that
can handle this.

There a media descriptor byte that need to be tweaked for this. Maybe
I'll remember the details?
